An unannounced case management visit was made to the facility by Licensing Program Analyst (LPA), Cindy Castro in response to a self-reported, incident that occurred at the facility on 05/16/25. In which it is alleged that a Staff( S1) picked up Child ( C1 ) by the neck and carried C1 across the classroom. During today’s visit, LPA met with Center Director, Staff and children to conduct interviews. The incident was self-reported and submitted as an Unusual Incident Report ( UIR) to the Department. Additional follow-up will be conducted. There were no Title 22 deficiencies were cited during today's inspection. A notice of site visit was given to facility representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the facility representative, Brenda Hardaway.
