What the inspector wrote

An unannounced Case Management visit was conducted at the facility by Licensing Program Analysts (LPAs) Selena Mariani and Jamie Clark in response to a self reported Unusual Incident Report (UIR) notification received by the department on January 7, 2026 regarding a child (C1) who was unsupervised in the playground. LPAs met with Center Director (CD) Brenda Hardaway and spoke with her about the purpose of the visit. According to the self reported UIR and interviews conducted with CD, Assistant Director (AD) and 4 staff (S1-S4), at approximately 10:33 AM on January 7, 2026, S1 saw C1 run to the play structure. S1 asked S2 if there was a child on the play structure. As S2 walked to the play structure, C1 was observed to be walking up the stairs and S2 picked C1 up after going down the slide. CD, S1 and S2 stated C1 was outside in the fenced in playground for approximately 2 minutes but less then 5 minutes. CD stated the classroom door with a doorbell that leads to the playground was intermittently working. CD stated on Wednesday, January 21, 2026 there is an all staff meeting/training on supervision and the classroom door bell was fixed. AD is working with inclusion services. The following violation of the California Code of Regulations, Title 22; Division 12, was observed: see LIC 809D. Appeal rights were provided. Report was reviewed and Center Director Brenda Hardaway. A notice of site visit was given and must remain posted for 30 days. Failure to do so shall result in an immediate civil penalty of $100.
