What the inspector wrote

An unannounced case management visit was made today by Licensing Program Analysts (LPAs) Robert Maciel-Kashima and James Clark for the purpose of following up on an Unusual Incident Report (UIR) submitted to the department on 4/1/26 regarding a lack of supervision. LPAs met with Director Brenda Hardaway. During today's visit, LPA's observed 40 children in care and interviewed Director. LPA's interviewed Director Brenda Hardaway who stated that on 3/25/26 between 10:00 a.m. and 10:05 a.m., she observed a child (C1) in the school age room left unattended by staff while the child's group was playing on the playground outside. Director states that the child was taken to the bathroom by staff but the staff member left the school age room to the outside without the child. The following violation of the California Code of Regulations, Title 22; Division 12, was observed: see LIC 809D. A civil penalty of $250 is being assessed on today's date for a repeat violation that occurred on 01/15/2026. Appeal rights were provided. This report was reviewed and Center Director Brenda Hardaway. A notice of site visit was given and must remain posted for 30 days. Failure to do so shall result in an immediate civil penalty of $100.
