What the inspector wrote

On 04/14/25 Licensing Program Analyst (LPA), Cindy Castro conducted an unannounced case management deficiency visit. LPA met with Center Director Justice Willis. During the course of an investigation, it was revealed through evidence received, that two staff were having an argument yelling derogatory terms while children were present. Center Director denies knowledge of the incident. LPA reminded Center Director of, Title 22 CCR, 101223 Personal Rights (a)The licensee shall ensure that each child is accorded the following personal rights:(1)To be accorded dignity in his/her personal relationships with staff and other persons. A plan of correction was discussed with Center Director. The following violations of the California Code of Regulations, Title 22; Division 12, were observed: see LIC 809D. Appeal Rights were provided and discussed. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted, and report was reviewed with Center Director, Justice Willis.

  • OBSERVATION OF THE CHILD

    101226.3 Observation of the Child (b)Any unusual behavior, any injury or signs of illness requiring assessment and/or administration of first aid by staff shall be reported to the child's authorized representative and recorded in the child's record. This requirement was not met as evidence by: Based on interviews that staff are not reporting incidents that occur to parents or child's authorize representative,which poses a potential Health, Safety and Personal Rights risk to children in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION FOR INFANTS

    101429 Responsibility for Providing Care and Supervision for Infants (a)In addition to Section 101229, the following shall apply:(1)Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement was not met as evidenced by: Based on interviews and photographic evidence that staff are not actively supervising children due to being on their cellphone, which poses a potential Health, Safety and Personal Rights risk to children in care.
