This requirement was not met as evidenced by: Based on interviews and photographic evidence that staff are not actively supervising children due to being on their cellphone, which poses a potential Health, Safety and Personal Rights risk to children in care.

    101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons.This requirement is not met as evidenced by: Based on evidence received in which two staff are having an argument, yelling derogatory terms while children are present, which poses a potential Health, Safety and Personal Rights risk to children in care.

    Based on staff record reviewed at 10:56am which revealed that S6's record was missing transcript. The licensee did not comply with the section cited above in count of one out of six staff which poses a potential health, safety or personal rights risk to persons in care.

  • PERSONNEL REQUIREMENTS

    Based on staff records reviewed at 10:56am which revealed S1-S6 were missing Health Screening & S1-S2's records did not contain evidence of negative TB clearances. The licensee did not comply with the section cited above for several staff which poses a potential health, safety or personal rights risk to persons in care.

  • STAFF-INFANT RATIO

    There shall be a ratio of one teacher for every four infants in attendance.This requirement is not met as evidenced by: Based on statement provided by AD and LPA’s observations of three staff providing care and supervision for 18 infants in the Infant classroom on 05/17/23 at 8:54am. This poses/posed an immediate health, safety and/or personal rights risk to the children in care.

    Staff shall place infants up to 12 month of age on their backs for sleeping.This requirement is not met as evidenced by: Based on LPA's observations of a child (C1) under 12 months old that was unable to climb out of the crib was laying on his/her side, covered with a blanket, and napping on a pillow/cushion on the ground. This poses/posed a potential health, safety and or personal rights risk to the child(ren) in care.

  • INFANT CARE ACTIVITIES

    If an infant falls asleep before being placed in a crib, staff shall move the infant to a crib as soon as possible.This requirement is not met as evidenced by: Based on LPA's observations of a child (C1) under 12 months old that was unable to climb out of the crib was napping on a pillow/cushion on the ground instead of a crib. This poses/posed a potential health, safety and or personal rights risk to the child(ren) in care.

    All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Obtain a California clearance or a criminal record exemption as required by the Department or This required is not met as evidenced by: Based on Department records, staff time sheets and statements provided by S1, CD and AD which corroborated S1 & S2 worked at the facility without a criminal record or exemption clearance; and a $1000 Civil Penalty was assessed, as this posed an immediate health, safety, and personal rights risk to the children in care.
