Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Selena Mariani made a visit to the facility for the purpose of delivering complaint investigation findings. During the infant license # 483001837 complaint investigation (Control Number 01-CC-20251217150238), additional deficiencies were identified and a case management report was added to the visit. LPA met with Acting Center Director (ACD) Arianna "Ari" Kalista. During a complaint investigation, LPA conducted interviews with parents, current and former adult staff (A1, A5, A7 & P1) which revealed that staff 1 (S1) placed 2 children (C1-C2) in a crib for punishment or time-out. The following violations of the California Code of Regulations, Title 22; Division 12, were observed: see LIC809-D. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Acting Center Director (ACD) Arianna "Ari" Kalista.
