Inspector’s narrative

What the inspector wrote

Staff interview (S1) stated upon her arrival to the facility at 8:30 am there were 14 children in care, being supervised by one staff member. At 9:28 am LPA observed one teacher (S2) with 23 students in a main classroom called the language room, while D1 went outside to the back of the facility to conduct other business for approximately 2 minutes until LPA requested D1 to return to the classroom. At 11:43 am LPA observed 24 children outside with one fully qualified (S2) teacher and one unqualified aide (S1). Based on the investigation, the preponderance of evidence standard has been met. Therefore, the above allegation is found to be substantiated. The following violations of the Health and Safety Code section 1596.895; see LIC 9099D. Appeal rights were provided. Exit interview was conducted, and report reviewed with Licensee Monika Verma. A notice of site visit was given and must remain posted for 30 days.

    (1) Except as specified in (3) below, good physical health shall be verified by a health screening, including a test for tuberculosis, performed by or under the supervision of a physician not more than one year prior to or seven days after employment or licensure.This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in which S1 did not have a health screening, or TB test which poses/posed a potential health, safety or personal rights risk to persons in care.

    (b) The person who brings the child to, and removes the child from, the center shall sign the child in/out.This requirement is not met as evidenced by: Based on records reviewed there were four children who were not signed in for care by their guardians on 06/25/2025. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

  • ARTICLE 1. General Provisions and Definitions

    (a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on record review, staff ( S1) was missing proof of immunity or vaccination against measles and pertussis. Licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

  • ARTICLE 2. Administration of Child Day Care Licensing

    (b) (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by:Based on record review, staff (S1) did not possess a current mandated reporter training certificate which poses a potential health, safety or personal rights risk to persons in care.
