Based on record review, the director did not comply with the section cited above by LPAs reviewed that 2 children files were missing the signed consent form for emergency medical treatment which poses a potential health, safety or personal rights risk to persons in care.

HEALTH-RELATED SERVICES

Based on observation and interview, the director did not comply with the section cited above by LPAs reviewed that incidental medical medication was stored at the facility for two children, however there were no incidental medical services plan developed for those children which poses a potential health, safety or personal rights risk to persons in care.

ARTICLE 2. Administration of Child Day Care Licensing

1596.41 Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request. This requirement is not met as evidenced by: Based on record review the facility roster was incomplete upon request. This poses a potential health, safety or personal right risk to persons in care.

ARTICLE 2. Administration of Child Day Care Licensing

(b) (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by:Based on record review, staff (S1) did not possess a current mandated reporter training certificate which poses a potential health, safety or personal rights risk to persons in care.

PERSONNEL REQUIREMENTS

(1) Except as specified in (3) below, good physical health shall be verified by a health screening, including a test for tuberculosis, performed by or under the supervision of a physician not more than one year prior to or seven days after employment or licensure.This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in which S1 did not have a health screening, or TB test which poses/posed a potential health, safety or personal rights risk to persons in care.

SIGN IN AND SIGN OUT

(b) The person who brings the child to, and removes the child from, the center shall sign the child in/out.This requirement is not met as evidenced by: Based on records reviewed there were four children who were not signed in for care by their guardians on 06/25/2025. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

ARTICLE 1. General Provisions and Definitions

(a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on record review, staff ( S1) was missing proof of immunity or vaccination against measles and pertussis. Licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

IMMUNIZATIONS

Based on record review, Of all 7 children's records reviewed, none of them had their immunizations on blue CDPH 286 form. Some had a copy of their immunizations, most did not. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
