What the inspector wrote

On 12/22/2025 at 09:45am, Licensing Program Analysts (LPAs), Jessica Gaumann and Robert Maciel made a case management inspection and met with Director,Tanika Hopkins (D1). The inspection was made in response to two Unusual Incident Reports (UIR), the first was received on 12/12/25 and the second was received on 12/18/25. During today's visit, LPAs reviewed child files to verify medical records and the facility medical policies, including Incident Medical Services (IMS) policy. IMS policy was discussed, including the requirement for facilities that have children enrolled with IMS to submit an IMS plan to the department. LPAs reviewed medication stored at the facility and reviewed the requirements for all stored medication and what documentation is required. PIN 22-02-CCP, Parent Consent for Administration of Medication form LIC9221 and the Centrally Stored Medications and Destruction Record form LIC622 were printed and given to D1. During interviews with D1 and staff (S1, S2) in regards to the UIR received on 12/18/25, it was revealed that, on 12/17/25, a child (C1) had gone to the bathroom alone without staff supervision. The following deficiency is being cited (see LIC 809D). Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Director, Tanika Hopkins.

Supervision shall include visual observation.The requirement was not met as evidenced by: Based on staff interviews, a child (C1) was allowed to use the bathroom on 12/17/25 by themselves without staff supervision which poses a potential risk to the health, safety, and personal rights of children in care.
