Inspector’s narrative

What the inspector wrote

Licensing Program Analysts (LPAs) Amy Strother and Jessica Gaumann arrived to the facility for the purpose of conducting multiple complaint investigations. When LPAs arrived they were greeted by staff (S1) at the office slider door. LPAs entered the infant room observing 6 children present, 2 in the play room and 4 in the nap room, with one staff, S1. S1 stated that the Licensee just left to go across the street to her other center. S1 called L1 back to the center. S1 and L1 stated that 3 of the children in care (C1-C3) are infants and the other 3 children (C4-C6) were preschool age. L1 stated that she is having a staffing shortage and needed to combine the ages today. Although the facility has a waiver on file to commingle infant and preschool aged children, it is to be only between 7:30am-8:00am and 5:00pm-5:30pm or for up to one hour 4:30-5:30pm, with staff-infant ratio of 1:4 infants and teacher-child ratios of 1:12. During a tour of the facility, LPAs observed multiple "rat & mouse killer" black boxes on the preschool yard. The boxes had labels that stated "do not allow children or pets to play with the bait station, if swallowed, call a poison control center, doctor or..." Based on interviews conducted, children have been observed playing on the play yard while the poison bait boxes were present in the play area. The following violation(s) of the California Code of Regulations, Title 22; Division 12, were observed: see LIC 809D. Appeal Rights were provided. A notice of site visit was given to licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Continue on LIC809-C Exit interview conducted and report was reviewed with licensee, Monika Verma. LPA Strother informed licensee Monika Verma that this report dated 11/05/25 documents one Type A citation. Type A citations shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Strother informed the licensee to provide a copy of this licensing report dated 11/05/25 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

    (e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (1) Obtain a California clearance or a criminal record exemption as required by the Department.This requirement ws not met as evidenced by: Based on interview and record review, two staff S1 and S2 were working with children at the facility without an associated criminal record clearance, which poses an immediate health & safety and personal rights risk to the children in care.

    (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below.(1) The number of children in attendance shall not exceed licensed capacity.This requirement was not not been met as evidenced by: Based on observation, LPAs entered the infant room observing 6 children present, 2 in the play room and 4 in the nap room, with one staff, S1, which poses a potential health & safety or personal rights risk to persons in care.

    (g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. (1)Storage areas for poisons shall be locked.This requirement was not not been met as evidenced by: Based on observation and interview, multiple rodent poison bait traps were observed on the preschool playground. Based on interview the traps were present when the playground was used by children on 11/04/25, which poses an immediate health and safety risk to children in care.

  • INDOOR ACTIVITY SPACE FOR INFANTS

    (b) Indoor activity space for infants shall be physically separate from space used by children in the child care center and school-age child care center components.This requirement was not not been met as evidenced by: Based on observation, LPAs entered the infant room observing 6 children present, 2 in the play room and 4 in the nap room, with one staff, S1, which poses a potential health & safety or personal rights risk to persons in care.
