What the inspector wrote

(Continue from LIC9099-C) The law enforcement case was closed as it was determined that no crime occurred. Other children and parent interviews did not report any other issues or concerns and no supervision issues were identified. Based on the IB investigation, although the allegation may have happened or is valid, there is not a preponderance of evidence to support the allegation. Therefore, the allegation is unsubstantiated. This report was read and reviewed with the Licensee. There were no Title 22 deficiencies cited related to this complaint allegation. Appeal rights were provided. The Notice of Site Visit shall be posted for 30 days.

    Based on record review, the facility did not comply with the section cited above. LPAs reviewed that 2 staff member's proof of immunizion, influenza, pertussis, and measles, were not on file which poses a potential health, safety risk to persons in care.

  • ARTICLE 2. Administration of Child Day Care Licensing

    Based on interview with Director & Staff record review, the facility did not comply with the section cited above. Director could not provide proof of completion of the mandated reporter training for all staff, which poses a potential health, safety risk to persons in care.
