IMMUNIZATIONS

Based on record review, the licensee did not comply with the section cited above. The licensee did not comply with the section cited above. LPA observed four children's Immunization Records were not transcribed onto the blue CDPH 286 form, which poses a potential health, safety or personal rights risk to persons in care.

ARTICLE 2. Administration of Child Day Care Licensing

Based on record review, the licensee did not comply with the section cited above, three staff did not have a current Mandated Reporter Training Certificate which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in, reviewed that multiple documents were missing children's authorized representative's signatures which poses a potential health, safety or personal rights risk to persons in care.

RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

Responsibility for Providing Care and Supervision. No child(ren) shall be left without the supervision of a teacher at any time This requirement is not met as evidenced by LPA observing child in an emply classroom on a table at 12:40pm on 2/26/25 without teachers knowledge. This poses a potential health and safety risk to the children in care.

BUILDINGS AND GROUNDS

101238(g) Buildings and Grounds: Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. This requirement is not met as evidenced by: Based on LPA's observation a disinfectant spray bottle located in the daycare children’s restroom and accessible to daycare children. In addition, LPA observed other cleaning supplies located on the kitchen counter with the door opened which poses an immediate health and safety risk to the children in care.

PRESCHOOL PROGRAM WITH TODDLER COMPONENT

101216.4(a)(2) Preschool Program with Toddler Component: Licensees serving preschool-age children may create a special program component for children who are between 18 months and 36 months of age...The toddler program shall be conducted in areas physically separate from those used by older or younger children...This requirement is not met as evidenced by: Based on LPA's observation three toddler aged children were commingling with the pre-school aged children in the pre-school classroom.

PERSONAL RIGHTS

Personal Rights-The licensee shall ensure that each child is accorded the following personal rights...To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature... This requirement was not met based on observation by LPA Ouye observing staff S1 yelling at toddlers which is a personal rights violation of the children in care.

ARTICLE 2. Administration of Child Day Care Licensing

Based on interview with Director & Staff record review, the facility did not comply with the section cited above. Director could not provide proof of completion of the mandated reporter training for all staff, which poses a potential health, safety risk to persons in care.
