REPORTING REQUIREMENTS
Reporting Requirements- upon occurrence of an unusual incident a report shall be made to the department by telephone within the next business day during business hour and a written report shall be submitted within 7 days following the occurrence. This requirement has not been met as evidenced by: the incident occurrence date was 1/30/25 and the department was notified with a unusual incident report on 02/18/2025.This poses a potential health and safety risk to the children in care.

Food Services-All food shall be safe and of the quality and in the quantity necessary to meet the needs of the children.This requirement was not met as evidenced by: staff interviewed indicated that milk was not served on one day in mid January 2025 dues to milk being spoiled and the program ran out of fresh fruit. This poses a potential health and safety risk to the children in care.

RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION
Responsibility for Providing Care and Supervision-No child(ren) shall be left without the supervision of a teacher at any time. This requirement was not met as evidenced by: child C1 was found outside of the outdoor preschool play area fencing but within the secondary fence based on staff admission on 1/30/25. The child was within audible range. This poses a potential health and safety risk to the child in care.
