(a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.This is not met as evidenced by: On 2/12/26, C1 was unsupervised in the classroom for approximately 1 minute. Staff did not observe C1 enter the classroom by themself which poses a potential risk to the health and safety or personal rights of children in care.

This requirement has not been met as evidenced by: Based on interview with staff S1 and video of child alone in outdoor play yard without any staff for 8 minutes on 4/9/25 which poses an immediate health, safety and or personal rights risk to the child in care.

Reporting Requirements- upon occurrence of an unusual incident a report shall be made to the department by telephone within the next business day during business hour and a written report shall be submitted within 7 days following the occurrence. This requirement has not been met as evidenced by: the incident occurrence date was 1/30/25 and the department was notified with a unusual incident report on 02/18/2025.This poses a potential health and safety risk to the children in care.

The child was within audible range. This poses a potential health and safety risk to the child in care.
