What the inspector wrote

A Case Management inspection was conducted at the facility by Licensing Program Analyst (LPA) Jen Patel in response to an Unusual Incident Report (UIR) received 2/13/26 involving a child left alone. During today's visit, LPA met with site supervisor, Paloma Escalante (SS) to discuss the incident and obtain additional details. During facility tour LPA observed 20 children in care being supervised by 4 staff. The facility director (D1) arrived a few minutes later. LPA provided guidance to D1 on licensing standards and regulations. During the visit, S1 reported that on 2/12/26, while the children were playing outside, S1 became distracted helping another child and did not see C1 enter the classroom by themself. SS and S3 also did not observe C1 enter the classroom. SS and S1 reported that C1 was discovered by themself in the classroom by S2 who was leading the groups transition back inside. SS, S1, and S3 also reported C1 was alone for one minute. Based on available information, it has been determined that inadequate supervision was not provided on 2/12/26 when C1 was left alone in the classroom. The following violation of the California Code of Regulations, Title 22; Division 12, was observed: see LIC 809 Deficiency page. Appeal Rights were provided. A notice of site visit was given to site supervisor, Paloma Escalante (SS) and must remain posted for 30 days. Exit interview conducted and report was reviewed with Paloma Escalante (SS).

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.This is not met as evidenced by: On 2/12/26, C1 was unsupervised in the classroom for approximately 1 minute. Staff did not observe C1 enter the classroom by themself which poses a potential risk to the health and safety or personal rights of children in care.
