What the inspector wrote
On 1/20/2026 an unannounced annual inspection was conducted by Licensing Program Analyst, (LPA) Yesenia Fierro. LPA met with Director, Connie Centeno and toured the facility, both indoors and outdoors. LPA observed all required licensing forms, menus, and daily activity schedules are posted in a visible location for authorized representatives to view them. This facility is a full day program, Monday through Friday from 8: 00a.m-2:00p.m.
The facility is composed of three (3) classrooms. Classrooms 1 and 2 are designated for preschool and classroom 3 is designated for children who are 2 years of age. During today\342\200\231s inspection a census was taken and there were 33 day care children present. During today\342\200\231s inspection LPA Fierro observed the facility has a complete first aid kit. The facilities, furniture, surfaces, floors, carpets and equipment, are in serviceable and sanitary condition. Toilets and handwashing facilities are safe, sanitary, and in operating condition. LPA observed the facility to have age-appropriate learning material, toys and play equipment accessible to children were clean and in operable condition. This facility receives all meals from the onsite cafeteria. LPA observed classroom kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors.
The facility has two (2) playgrounds, classroom 3 has a designated playground behind the classroom. LPA Fierro observed playgrounds equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around high climbing equipment, such as swings and slides have cushioning material to absorb falls. There are no swimming pool or other bodies of water on the premises. CON'T 809-C
LPA reviewed a sample of children\342\200\231s files and observed files were complete with contact information for authorized representatives and or relatives or others who can assume responsibility for the child, medical assessment, and immunizations recorded. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. There is a ratio of one teacher supervising no more than 12 children in attendance. Capacity and limitations as specified on the license are being maintained. Staff are fingerprinted as a condition of employment through
Empire Union School District.
LPA reviewed a sample of staff files and observed files complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. Staff records contain documentation of meeting qualification requirements. The name of the childcare center director or fully-qualified teacher(s) designated to act in the director\342\200\231s absence has been reported to the Department.
Incidental Medical Services (IMS) are currently being provided. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed licensee Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infants devices with the CPSC to be notified of any recalls on their purchased equipment. CON'T 809-C
Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
CCC completed testing and no lead exceedances
assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.
Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.
