What the inspector wrote

On July 9, 2025, Licensing Program Analyst (LPA) Miguel Herrera and Licensing Program Manager (LPM) Jose Penate conducted an unannounced Case Management Inspection and met with Early Childhood Education (ECE) Coordinator, Kimbra Houck. The purpose of this inspection was to serve the facility with a Letter of Immediate Exclusion for staff #1 (S1) and staff #2 (S2), which informs the facility that S1 and S2 are excluded from being present in all facilities licensed by the Department. ECE Coordinator Houck was advised that the facility must ensure that S1 and S2 are to be immediately excluded from having contact with preschool children and from being physically present from all facilities licensed by the Department. ECE Coordinator Houck was advised that the exclusion order remains in place, unless otherwise notified in writing by the Department. ECE Coordinator Houck stated that the Burbank Head Start Program will be closed for the following school year and confirmed that the facility is currently closed for summer break therefore S1 and S2 are not currently working. LPA Herrera provided ECE Coordinator Houck with information/rights to appeal the Department\342\200\231s order within 15 days of today\342\200\231s service notification. ECE Coordinator, Kimbra Houck is aware that S1 and S2 cannot return to the facility and by signing this report, she acknowledges receipt of the Letter of Immediate Exclusion notifying the facility that S1 and S2 are immediately excluded from all facilities licensed by the Department. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiency is being cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with ECE Coordinator, Kimbra Houck. Appeal rights were provided. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
