What the inspector wrote

Licensing Program Analyst (LPA) Aurelio Mendoza conducted an unannounced Annual/Random Inspection and was met by Licensee Rosalina Carillo, who primarily speaks Spanish. LPA assisted with interpretation throughout the visit. The licensee confirmed that she is currently providing daycare services Monday through Friday, from 7:00 a.m. to 4:00 p.m. LPA toured the home inside and outside, and a census was taken. The current facility sketch was reviewed, and it was confirmed that the kitchen, daycare room, living room #2, hallway bathroom, and the backyard are used for care and are accessible to children. During the visit, Licensee Rosalina Carillo requested that living room #1 be inspected for immediate use. LPA observed that the room met Title 22 requirements and approved its use. Both LPA and Licensee initialed and dated the updated facility sketch to reflect all areas accessible to children in care. All other rooms are off-limits and made inaccessible with doorknob spinners. However, a few rooms did not have child safety locks or devices in place. Licensee stated she will obtain safety locks as soon as possible. LPA issued a technical violation for this observation and reminded the licensee that while supervision is critical, physical barriers are also required to prevent children from entering unauthorized areas. Licensee stated that she has two outdoor cats that do not enter the home. LPA reminded her that in the event of an injury involving the animals, she would be responsible for their actions. There is no swimming pool or other bodies of water on the premises. Although no in-ground pool is present, LPA discussed the new pool safety requirements under AB 2866, effective 01/01/2025, and referenced PIN 25-01-CCP issued on 01/06/2025 for future reference. Licensee stated there are no firearms or ammunition on the premises. All poisons are kept in a locked storage area and no poisons were observed at the time of the inspection. However, LPA observed one lower kitchen cabinet with accessible cleaning products. A technical violation was issued, and the licensee was advised to install a child safety lock to ensure hazardous items remain inaccessible to children in care. The home has one fireplace located in the living room, which is secured by metal doors and will not be used during daycare hours. A working fire extinguisher, smoke detector, carbon monoxide detector, and adequate heating and ventilation were observed. The home is a single-story residence with no stairs. Age-appropriate toys and play equipment were available, and the home has working telephone service, which LPA verified. The outdoor play area is fenced; however, a technical violation was issued due to gates within the backyard leading to areas with gardening tools that were not properly secured, posing a potential hazard if accessed by children. Licensee maintains supervision of children at all times and is aware that children may not be left in parked vehicles. The facility is operating within the licensed capacity. A sample of children\342\200\231s records was reviewed. LPA found that several files were missing required documentation, including the LIC 627 Consent for Emergency Medical Treatment. A Spanish master packet of required Child Care Licensing forms was provided during the visit, and the licensee was given guidance on which forms must be completed and maintained in each child's file upon enrollment. A technical violation was issued. Licensee\342\200\231s Mandated Reporter Training is current through 01/16/2026. However, her pediatric CPR/First Aid certification expired on 03/18/2025, and a technical violation was issued. A review of staff records confirmed immunizations for influenza, pertussis, and measles are on file. LPA discussed the Community Care Licensing Division website ( www.ccld.ca.gov ), which offers access to forms, regulations, Provider Information Notices (PINs), and quarterly updates. Reporting requirements under Section 102416.2 were also reviewed. LPA reminded the licensee that all adults aged 18 and older living or working in the home must have a criminal record clearance or exemption on file prior to presence in the home. Failure to comply may result in civil penalties of $100/day for up to 5 days, or 30 days for repeat violations. Safe sleep regulations were discussed, and LPA referred the licensee to the Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep . During the review, it was noted that safe sleep logs were missing for a child under 24 months; this was documented as a technical advisory, and the licensee was advised to immediately begin maintaining these logs as required. LPA also encouraged the licensee to check the CPSC.gov website for recalled infant devices and to register infant equipment to receive product recall alerts. The licensee reported that there are currently no children in care requiring Incidental Medical Services (IMS). LPA reviewed IMS policies and referenced PIN 22-02-CCP. LPA also provided ADA information and shared a link to the U.S. Department of Justice publication: Commonly Asked Questions About Child Care Centers and the ADA . Licensee Rosalina Carillo was also informed about MyChildCarePlan.org, a consumer education website designed to connect families with child care providers and local Resource and Referral agencies. An exit interview was conducted and the report was reviewed with Licensee Rosalina Carillo. She confirmed there are no registered sex offenders residing at the home. LPA verified the RSO profile in FAS. Per Title 22, Division 12 of the California Code of Regulations, no deficiencies are cited. Licensee Rosalina Carillo was provided with appeal rights. This report shall be made available to the public upon request. LIC 9213 - Notice of Site Visit was issued and is required to be posted for 30 days.
