What the inspector wrote

An unannounced case management inspection was conducted today at 12:00pm by Licensing Program Analyst (LPA), Tammy Dutra. LPA met with facility representative Melissa Murphy-Correa. In response to an Unusual Incident Report received by the Department on 1/13/26. A child in care, C1 was playing in the sandbox and cut the inside of their mouth while in care. The facility representative was interviewed on 2/5/26 at 1 2:01 p m and stated that on 1/9/26 at 11:10am a child in care, C1 was playing in the sandbox and walked towards a play kitchen. C1 put their foot on the toy kitchen and then tripped falling forward. C1 was bleeding and staff immediately provided first aid. It was determined C1 had a large deep cut inside of their bottom lip. C1 was picked up from care shortly after the incident and the child's parent sought medical attention. During the incident there were fifteen children present with three staff members. Facility was operating within capacity and ratio requirements. C1 returned to school on 1/13/26. One staff was interviewed on 2/5/26 and stated that C1 was stepping on the play kitchen located in the sandbox and turned after being told not to climb on the kitchen, losing their footing and falling. S1 did not see what C1 fell on but immediately recognized C1 was crying and blood was visible on the child. S1 provided first aid and asked the family advocate to call the parent. Parent arrived shortly after the call and sought medical care for C1. One parent, P1 was interviewed on 1/29/26 and stated that C1 was doing fine and they had no concerns with regards to their child's care and supervision. P1 said the doctor stated C1 did not need stitches based on the location of the injury. C1 was given instructions to do mouth rinses and restrict diet to soft foods. P1 stated C1 returned the following week with no further issues. During today\342\200\231s inspection, the facility was toured and the playground was toured. Children's sandbox was photographed. LPA observed age appropriate toys. Based on interviews, staffing/ ratio reviews and observation it was determined that the incident was an accident and could not have been prevented. There were no deficiencies cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with the facility representative Melissa Murphy-Correa. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
