What the inspector wrote

On 11/13/2025, Licensing Program Analyst (LPA) Claribel Soto and Licensing Program Manager (LPM) Scott Herring conducted an unannounced Case Management \342\200\223 Incident Inspection. LPA and LPM met with Director Leanne Cerda. LPA took a census. The purpose of today's inspection was to follow-up on an unusual incident that was reported to Community Care Licensing (CCL) on 10/28/2025. The incident reported was regarding child #1 being left outside unsupervised for approximately 10 minutes on 10/28/2025. During today's inspection, LPA Soto and LPM Herring interviewed Director Leanne Cerda. The other staff members who were present during the incident were not present during the inspection but the details of the incident along with information provided during today\342\200\231s interview support that child was left on the outside play area and unaccounted for approximately 10 minutes. It was agreed that child #1 experienced some degree of adverse trauma as a result of being left outside alone. When the child was discovered by staff, he was observed crying and that he had urinated in his pants. Details of circumstances that led up to the event are as follows: The Unusual Incident was regarding child (child #1) who was left outside when children were transitioning back inside. Staff #1 saw child #1 under the play structure and let staff #2 know but did not hear staff #1. Staff #2 realized child was not in classroom and opened the door and saw child outside the door crying with wet pants. Incident occurred in the afternoon. Director stated that staff brought child inside, changed him and calmed him. Director stated that staff let parents know about the incident when the child was picked up. Continued on 809-C It is noted that the staff responded and attended to the child when the incident was discovered. They took steps to provide immediate support and are in the process of implementing corrective steps to account for children at all times moving forward. Director responded that she is willing to provide a written corrective plan to ensure care and supervision best practices and to participate in available trainings. Per California Code of Regulations Title 22, Division 12, Chapter 1, the following deficiency is cited (see next page). An exit interview was conducted with Director, Leanna Cerda.

    Responsibility for providing care and supervision: No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidence by child#1 was left outside and unaccounted for approximately 10 minutes. As a result the child was observed crying when staff discovered he was unaccounted for. This is an immediate health and safetyy risk.
