What the inspector wrote

On 04/22/2025, Licensing Program Analyst (LPA) Octavia Nolan conducted an unannounced case management inspection at the facility. LPA met with Facility Representative Xee Lathrop and explained the purpose of the inspection. LPA toured the facility inside and outside and took a census. On 04/14/2025, the facility notified Community Care Licensing Office (CCL) via telephone regarding an Unusual Incident. On 04/11/2025, during the outdoor activity time, Child #1 (C1) tripped while climbing the steps of the preschool play structure. The child tumbled backwards and hit their forehead on the edge of a step, resulting in a cut above the right eyebrow. Staff #1 (S1) witnessed the incident and immediately administered first aid to the child. Staff #1 applied pressure on the cut to stop the bleeding and an ice pack was also applied. Staff #2 (S2) called the child\342\200\231s parents and the ambulance due to the severity of the cut. The ambulance arrived within 3-5 minutes and the child\342\200\231s parent arrived shortly after. The child was taken to Sierra View Medical Center Emergency Room by the parent and received stitches for the facial laceration. The child is doing better and returned to care on 04/14/2025 with no restrictions. LPA inspected the area where the incident took place and interviewed staff. The play structure has age-appropriate stickers for 2-5 year old children and no hazards were observed. LPA recommended the facility increase staff zoning near the play structure steps. This appears to be an isolated incident and staff took appropriate measures to address the child\342\200\231s injury. The facility also followed appropriate reporting requirements required by the Department. Exit interview conducted and report was reviewed with Facility Representative Xee Lathrop. Appeal rights Continued on LIC 809C were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
