What the inspector wrote

On 02/22/2022, Licensing Program Analyst (LPA) Pete Espinoza arrived at the facility unannounced to complete the investigation into the above allegations. LPA met with Holly Oliveira, Licensee. and toured the facility. LPA explained the reason for this inspection with Director and census was taken. LPA interviewed Director. Based upon observations and information gathered through interviews and a review of records, the Licensing agency has determined the preponderance of evidence standards has been met, therefore, the above allegations are found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter (1/3), are being cited on the attached LIC 9099D. An exit interview was conducted with Holly Oliveira, Licensee, a plan of correction was discussed, and appeal rights were explained. A printed copy of this report as well as a printed copy of the appeal rights was provided to Licensee at the conclusion of the visit. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS.

  • REPORTING REQUIREMENTS

    Reporting Requirements: Upon the occurrence, during the operation of the child care center... a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. This requirement is not met as evidenced by interviews and records review conducted during today’s complaint investigation. Licensee failed to notify Fresno Regional Office of outbreak of a communicable disease. This poses a potential risk to the health, safety or personal rights of children in care.

  • PERSONAL RIGHTS

    Personal Rights: a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement is not met as evidenced by interviews, observations conducted during today’s complaint investigation. LPA observed that masks were not being worn by staff and/or children inside the center during today's complaint inspection. This poses an potential risk to the health, safety or personal rights of children in care.
