What the inspector wrote

On 12/04/2025, Licensing Program Analyst (LPA) Octavia Nolan conducted an unannounced case management inspection for the preschool license. LPA met with Licensee Holly Oliveira and discussed the purpose of the visit was to ensure compliance with Title 22 regulations. LPA took a tour of the facility and a census. During the inspection, LPA conducted a physical inspection and observations. No poisons were observed during the inspection. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. Furniture and equipment are in good condition. Playground equipment is in safe condition. The facility recently had grass turf installed on the preschool playground and the area is in a safe condition and free of hazards. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. All toilets and handwashing facilities are in safe and sanitary operating condition. Facility has one or more functioning carbon monoxide detectors. Floors in the facility are clean and safe. Solid waste storage containers have tight-fitting covers and are in good repair. LPA verified all staff have obtained a criminal record clearance or exemption. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. The facility has completed quarterly all staff meetings during 2025 and also participated in the Technical Support Program (TSP). Exit interview conducted and report was reviewed with Licensee, Holly Oliveira. Appeal rights were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

    RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    (a) The licensee shall provide care and supervision as necessary to meet the children's needs. This requirement was not met as evidenced by: based on observation, interviews, and records review, Staff #1 did not ensure children were at a safe distance prior to closing the door on a child's finger causing a severe injury. Staff also did not secure the door closed after the incident, allowing children to exit to the playground without a teacher present which poses an immediate health, safety or personal rights risk to persons in care.
