What the inspector wrote

On 05/02/2025, Licensing Program Analyst (LPA), Sonja Navarrette and Licensing Program Manager (LPM) Luisa Gavoutian conducted a Case Management inspection. The purpose of the inspection was to follow-up on an unusual incident report that was reported to the Department on 04/04/2025. Incident was regarding Child #1 (C1) broke their clavicle on the playground. It was reported there were seven teachers on the play yard providing supervision however no one witnessed how the child sustained the injury. Today, LPA Sonja Navarrette and LPM Luisa Gavoutian met with, Director Blair Vendenoff, toured the facility and took a census of the children. LPA and LPM interviewed staff who reported on 4/4/25 at approximately 12:00PM Director was notified that C1 was pushed off the dome play structure by another child. C1 parents were notified, and first aid was administered. C1\342\200\231s parents picked up child and follow up at Emergency Department the next day. C1 was diagnosed with broken clavicle. In regard to the incident, LPA obtained information during the interviews that the incident was an isolated. LPA and LPM were informed that Director had followed the facilities\342\200\231 procedures to ensure the child\342\200\231s needs were being met. During the inspection LPA and LPM observed the dome where incident occurred measured approximately 2ft-2.5 ft in height. LPA and LPM also observed sufficient cushioning underneath the dome. Facility followed proper first aid and reporting procedures. Findings were reviewed with Director. Per California Code of Regulations, Title 22, Division 12, no deficiency cited. Exit interview conducted with Director, Blair Vendenoff. A copy of this report and Appeal Rights were provided and discussed with Director.
