Based on staff record review, the licensee did not comply with the section cited above. LPA observed that Lead Teacher of the Toddler Component and Facility Director were missing 3 units in the infant/toddler category which poses a potential health, safety or personal rights risk to persons in care.

Based on LPA observation the licensee did not comply with the section cited above. LPA observed children using the restroom without supervision on three separate occasions during the facility inspection. LPA also observed a child eating at a table unsupervised while teachers were with other children in another area of the classroom. Based on these observations, this poses an immediate health, safety or personal rights risk to persons in care.

BUILDINGS AND GROUNDS - The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidenced by observation. LPA Marquez observed a wooden gate with a broken/loose latch. This poses a potential risk to the health, safety and personal rights to children in care.

RESPONSIBILITY FOR PROVIDING CARE & SUPERVISION- The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by Director self reported an incident on 12/15/2022 of a child exiting the playground through a gate and walked onto the front facility parking lot unsupervised. This poses an immediate risk to the health, safety or personal rights of children in care.
