What Happened
Annual inspection revealed multiple facility maintenance issues including unlocked staff restroom with cleaning solutions, water-damaged walls from AC leak, playground hazards with broken windows and exposed nails, inadequate fall cushioning, carpet and mat tripping hazards, and unsanitizable napping mats. Staff mandated child abuse training certificates were expired.
Observations
- Staff restroom unlocked with cleaning solutions accessible to children
- AC unit leaking on exterior wall causing wood rot
- Playhouse with broken window grid, jagged edges, missing paint, and nails sticking out
- Gate behind playhouse not locked/closed
- Green metal pole with rust behind play yard
- Fencing with spider webs and nails
- Side fencing with nail points sticking out
- Artificial turf not glued to wooden square (tripping hazard)
- Insufficient cushioning material under climbing equipment and slides
- Ramp showing rust and chipping paint
- Black rugs with curling edges (tripping hazard)
- Air vents with black dust/dirt
- At least three napping mats with exposed foam (cannot be sanitized)
- Changing mat with torn corner and exposed foam
- AM snack (granola bar) did not match posted menu (bagel and cream cheese)
- Staff using Pinesol while children napping in area
- Two staff members with expired Mandated Child Abuse Training certificates

Based on observation, the licensee did not comply with the section cited above. During Observation LPA observed fencing sections with nail points and a rusted metal pole in a accessible area to children in care which poses a potential health, safety or personal rights risk to children in care.

Based on observation, the licensee did not comply with the section cited above. During inspection LPA observed a playhouse that had a grid broken, and nails in the side of the windows. LPA also observed AC leaking, rust in waterline that leads to toilet bow, carpet edges that were curled, loose artificial grass and a wooden gate that will not close/lock, which posed a potential health, safety or personal rights risk to persons in care.
