What the inspector wrote

On January 29, 2026, Licensing Program Analyst (LPA) Xona Xayavong conducted an unannounced case management\342\200\223incident inspection. LPA met with Office Manager Julia Isquierdo. LPA toured the facility inside and outside, and a census was taken. The purpose of today\342\200\231s inspection was to follow up on an unusual incident report received by the Fresno Regional Office on 12/18/2025. On December 18, 2025, Child #1 (C1) began running in the classroom and accidentally bumped into the corner of a shelf, resulting in a laceration on the child\342\200\231s forehead. Staff #1 (S1) reported that C1 had an assigned one-on-one staff member present on the day of the incident and that the incident was observed. Staff assessed the situation to determine whether emergency services were needed and provided first aid to the child. S1 stated the laceration on C1\342\200\231s forehead was approximately the size of a pencil eraser. C1 was picked up by a relative, and it was recommended that the child be taken to urgent care for possible stitches to close the wound. During today\342\200\231s inspection, LPA conducted interviews with staff, reviewed staff and children\342\200\231s files, and obtained facility documents. After gathering information, LPA determined that staff took appropriate action during the incident and have placed corner protectors on the shelves. Additionally, staff confirmed there is a Behavioral Intervention Plan (BIP) in progress to respond to the child\342\200\231s behavior if needed. An exit interview was conducted, and the report was reviewed with Office Manager Julia Isquierdo. Appeal rights were provided. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit was provided and is required to be posted for 30 days.
