What the inspector wrote

On 10/30/2025, Licensing Program Analyst (LPA) Octavia Nolan conducted an unannounced case management inspection at the facility. LPA met with Site Supervisor Donna Pascual and explained the purpose of the inspection. LPA toured the facility inside and outside and took a census. On 09/19/2025, the facility notified Community Care Licensing (CCL) via telephone of an unusual incident. On 09/18/2025 at approximately 11:25 AM, Child #1 (C1) was running during the outdoor activity time. Child #1 ran toward an equipment cart and hit their forehead on the cart basket. The basket is at the child's eye level. The child sustained a minor cut above their right eyebrow and a small bump in the middle of their forehead. Staff cleaned the cut and applied a band aide. The Site Supervisor called the child's parents regarding the incident and the child was picked up at 12:08 PM. The facility was notified by the parents later that evening that the child was taken to Sierra View Hospital Emergency Room and the cut was treated with skin glue to prevent scarring. The child returned to care on 09/19/2025 with a doctor note stating the child has no restrictions. During today’s inspection, LPA reviewed video footage of the incident and interviewed the Site Supervisor regarding the incident. LPA inspected the equipment cart and determined it was in good condition with no sharp, loose, or pointed parts. This appears to be an isolated incident and staff took appropriate measures to address the child’s injury. Exit interview conducted and report was reviewed with Site Supervisor Donna Pascual. Appeal rights were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
