What the inspector wrote

On 03/04/26 Licensing Program Manager (LPM) and Licensing Program Analyst arrived at the arrived at the facility to conduct an unannounced case management inspection. LPA met with Licensee, Mericia Garcia and a census was taken. The purpose of today\342\200\231s inspection was to address a complaint received by the Fresno Regional Office on December 15, 2025. The licensee was aware of the complaint allegation but failed to submit an Unusual Incident Report (UIR) by the next business day as required. LPA reviewed and explained the reporting requirements to the licensee. A Type B deficiency will be cited. Per Title 22, Division 12, of the California Code of Regulations, the following deficiency is being cited: (see next page). Licensee, Mericia Garcia was provided appeal rights. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
