California Code § 101419.2(b)(2)
Infants up to 12 months of age shall have a completed Individual Infant Sleeping Plan [LIC 9227 (3/20)], which is incorporated by reference.

- Using a generic sleep policy instead of the specific LIC 9227 form. Providers create their own infant sleep questionnaires and assume they satisfy the requirement. Inspectors look for the exact state form number and revision date. Your custom form doesn't count.
- Completing the form but not updating it as the infant grows. A plan filled out at 3 months old may not reflect changes by 9 months. Inspectors check whether the information matches the child's current age and developmental stage.
