Applicant Wilson Arcinue has spent the night in one of the resident rooms, LPA found his personal items in the closet of the home. Applicant is not fingerprint cleared.

    Based on observation and record review, the licensee did not comply with the section cited above in two out of two residents which poses an immediate health, safety or personal rights risk to persons in care. Two residents were observed to have been fed crushed medications in applesauce and no orders were found for doing so.

    Based on observation, the licensee did not comply with the section cited above in 5 out of 5 residents which poses an immediate health, safety or personal rights risk to persons in care.

  • Report serious outbreaks and major accidents

    (a) Each licensee shall furnish to the licensing agency such reports as the Department may require, including, but not limited to, the following:(2) Occurrences, such as epidemic outbreaks, poisonings, catastrophes or major accidents which threaten the welfare, safety or health of residents, personnel or visitors, shall be reported within 24 hours either by telephone or facsimile to the licensing agency and to the local health officer when appropriate. This requirement was not met as evidenced by no Incident Reports were submitted to Licensing for any of the 2 residents that tested positive for Covid or the additional 2 residents that were tested due to symptoms, which poses a potential risk to the health, safety, or personal rights risk to the residents in care.
