Basic services requirements. Every facility required to be licensed under this chapter shall provide at least the following basic services:Monitoring the activities of the residents while they are under the supervision of the facility to ensure their general health, safety, and well-being. Licensee failed this by a resident leaving the facility through the front door and was not found for multiple hours. This is an immedate health and safety risk to residents

Personal Rights of Residents in All Facilities. Residents in all residential care facilities for the elderly shall have all of the following personal rights: To have their representatives regularly informed by the licensee of activities related to care or services, including ongoing evaluations, as appropriate to their needs. This requirement is not met as evidenced by: Based on LPA's interviews the facility did not communicate the missing item to the responsible party. This does not pose an immediate risk to residents.

Care of Persons with Dementia. Licensees who accept and retain residents with dementia shall be responsible for ensuring the following:Each resident with dementia shall have an annual medical assessment as specified in Section 87458, Medical Assessment, and a reappraisal done at least annually, both of which shall include a reassessment of the resident’s dementia care needs. This requirement is not met as evidenced by:Based on LPAs interivews everyone knew resident required dentures but it was not written down. This does not pose an immediate risk to residents.

Managed Incontinence - In addition to Section 87611, General Requirements for Allowable Health Conditions, the licensee shall be responsible for the following: Ensuring that incontinent residents are kept clean and dry and that the facility remains free of odors from incontinence. LPA witnessed the facility smelling of urine. The licensee did not ensure that the facility remains free of odors from incontinent residents. This poses a potential safety risk to residents in care.

87625 (b)(5) Managed Incontinence - In addition to Section 87611, General Requirements for Allowable Health Conditions, the licensee shall be responsible for the following: Ensuring that the appropriately skilled professional developing the bowel and/or bladder program provide training to facility staff responsible for implementation of the program. The licensee did not ensure that training was provided by a skilled professional for staff that are providing incontinence care to residents. This poses an immediate safety risk to residents in care.

87465 Incidental Medical and Dental Care (h)(6) The licensee shall be responsible for assuring that a record of centrally stored prescription medications for each resident..This requirement was not met as evidenced by Based on statements and documents, Licensee did not maintain an accurate centrally stored medication log for 1 of 1 resident which poses a potential health and safety risk to resident in care.
