874763 Reappraisals (a) The pre-admission appraisal...Section 87457, Pre-Admission Appraisal, shall be updated in writing as frequently as necessary...to note significant changes in condition...and to keep the appraisal accurate...reappraisal.This requirement is not met as evidenced by: The licensee did not comply with the above cited section, as R1 was admitted to hospice care, a new physician's report indicates R1 required additional care, however, no reappraisal was completed, which posed a potential health risk to residents in care.

This requirement is not met as evidenced by: Based on interviews and records review, the licensee did not comply with the section cited above. Facility staff did not respond to R1 in a timely manor

Personal assistance and care for required daily activities

Basic Services. Basic services shall at a minimum include: Personal assistance and care as needed by the resident and as indicated in the pre-admission appraisal … such as dressing, eating, bathing… This requirement is not met as evidenced by: Based on interviews, R1 did not receive three showers a week as indicated in their needs and services plan. This poses a potential health and safety risk to residents in care.

Heat occupied rooms to minimum temperature

87303 (b)A comfortable temperature for residents shall be maintained at all times.(1) The facility shall heat rooms that residents occupy to a minimum of 68 degree F, (20 degrees C).This requirement is not met as evidenced by:Based on observation the temperature inside resident 1's (R1) room observed at 58 degrees Fahrenheit. Six (6) out of eight residents interviewed reported facility temperature is not maintained and it is too cold. This poses/posed a potential health, safety and personal rights risk to residents in care.

This requirement was not met as evidenced by: Based on 6 out 8 resident interviews and record review, response times to calls for assistance have taken from 10 minutes to an hour. in some instances.

Fire approval and staff access to unlock systems

Based on observation, the licensee did not comply with the section cited above in three out of ten resident rooms LPA observed over-the-counter medication in two rooms and room deodorizer in one room that were in unlocked cabinets, which poses an immediate health, safety or personal rights risk to persons in care.
