R1 expressed that they hit their head but staff did not seek medical attention for R1 which posed a potential health risk to clients in care.

87705 Care of Persons with Dementia(d) The licensee shall ensure that the facility has an auditory device or other staff alert feature to monitor exits on exterior doors and perimeter fence gates...This requirement is not met as evidenced by: Based on observation the licensee did not comply with the section cited above as exit doors throughout the facility were observed to be equipped with auditory alarms that were turned off and did not alert when opened which poses a potential safety risk to clients in care.

(h)(2) The following requirements shall apply to medications which are centrally stored: Centrally stored medicines shall be kept in a safe and locked place that is not accessible.This requirement is not met as evidenced by: Based on LPA's observations, the licensee did not comply with the section cited above as resident medications were not kept in a safe locked place which posed an immediate health risk to persons in care.

87705 Care of Persons with Dementia (f)(2) The following shall be stored inaccessible to residents with dementia: Over-the-counter medication, nutritional supplements or vitamins, alcohol, cigarettes, and toxic substances such as certain plants, gardening supplies, cleaning supplies and disinfectants.This requirement is not met as evidenced by: Based on LPA's observations, the licensee did not comply with the section cited above as toxic substances were observed throughout the facility accessible to residents which posed an immediate health risk to persons in care.
