This requirement was not met as evidenced by:Based on the LPA's interview with the center director on 04/28/23, the center director stated that the facility did not obtain signed LIC9224 “Acknowledgement of Receipt of Licensing Reports” from parents and guardians of each child receiving services in this facility. The facility was cited a Type A violation on 01/04/23. This poses a potential health and safety risk to children in care.

This requirement was not met as evidenced by: Based on interviews with facility staff on 04/28/23, it was corroborated that due to a lack of supervision, child C1 cut their hair with a pair of children's scissors found in the classroom; this was not observed by any staff member.
