Case management investigation of two self-reported incidents. Type A violation cited for staff inappropriately handling a child by grabbing by the arm with no injuries reported. Staff member involved no longer employed by facility.

Incident investigation: teacher showed horror film character to children causing one child to become frightened. Type B deficiency cited.

Licensing conducted an unannounced case management inspection after reviewing documentation about an incident involving staff handling of a child. The narrative states that one teacher tried to stop a child from running in the classroom, another teacher intervened, grabbed the child, placed the child on the floor, and held the child down with a hand on the child's back. A Type A deficiency was cited.

Child left assigned classroom area and accessed bathroom alone without adult supervision. Incident observed by Federal Head Start monitor during facility tour.

Lead testing case management: two faucets identified with elevated lead levels by certified independent water sampler. Facility conducted tour and documentation review.

Case management investigation of self-reported unusual incident determined that staff member physically moved child on mat multiple times, causing child to fall during one transition.
