    During records review LPA Herrera observed that 4 out of 7 staff members were either missing immunizations records or had incomplete immunization records on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by staff not supervising preschool children in restrooms and leaving children in back classroom while all staff are in the front classroom, which poses a possible threat the the health and safety or personal rights to children in care.

  • PERSONAL RIGHTS

    (a) The licensee shall ensure that each child is accorded the following personal rights:(3) To be free from corporal or unusual punishment... humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions... This requirement was not met as evidence by children were left in time out in a classroom by themselves and staff # 1 (S1) was observed speaking innapropriately to and roughly grabbing children in care, which are immeidate threats to the health and safety or personal rights to children in care.

    Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed 19 children in Advanced Preschool classroom with 1 teacher and 1 aide. Upon reviewing files LPA could not find any units for the aide. LPA spoke with aide and aide stated she did not have any units so therefore they are over ratio which poses a potential health, safety or personal rights risk to persons in care.

  • SIGN IN AND SIGN OUT

    Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed in 2 classrooms 6 parent signatures were missing from the sign in sheet which poses a potential health, safety or personal rights risk to persons in care.

  • ARTICLE 2. Administration of Child Day Care Licensing

    Based on observation, interview, and record review, the licensee did not comply with the section cited above. 1 staff has not completed the mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

  • CRIMINAL RECORD CLEARANCE

    During file review and interviewing staff it was confirmed that both staff present today did not have a fingerprint clearance nor were assocaited to the preschool license which poses an immediate health, safety or personal rights risk to persons in care.

    Based on interviews & records review It was confirmed that both staff present during todays inspection did not comply with the section cited above since they do not have a CPR First Aid Certificate which poses a potential health, safety or personal rights risk to persons in care.
