RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

101229(a)(1) Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary...No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation.This requirement was not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in that it was determined that on 6/13/24 there was an Absence of Supervision resulting in C1 leaving the facility unsupervised, which poses an immediate health and safety risk for persons in care.

101226 Health-Related Services (a)The licensee shall immediately notify the child's authorized representative... or...(2)In the case of less serious injuries...notify the child's authorized representative of the nature of the injury when the child is picked up from the center. This requirement is not met as evidenced by: Based on interviews and record reviewed, the licensee did not comply in staff admitted in not notifying the authorized representative of a child’s injury at time of pick-up, which poses a potential Safety and Personal Rights risk to persons in care.

FIXTURES, FURNITURE, EQUIPMENT AND SUPPLIES

Based on observation and interview, the licensee did not comply with the section cited above in 31 out of 31 persons, as LPA observed a large wooden playhouse on the playground that had several areas of wood that was in disrepair which poses a potential health, safety or personal rights risk to persons in care.

ARTICLE 2. Administration of Child Day Care Licensing

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 6 persons, as Mandated Reporter Certification for Site Director expired on 4/28/2021which poses a potential health, safety or personal rights risk to persons in care.
