This requirement was met as evidenced by: Review of records and observation showed S1 speaking to children in an inappropriate tone and manner. This poses a potential risk to the health, safety, and personal rights of children in care.

This requirement was not met as evidenced by: Record review documented that Staff 1 (S1) physically handled infants in a rough and punitive manner during aggressive time-out placements. Staff 1 (S1) and Staff 2 (S2) failed to console or comfort crying infants, demonstrating a failure to meet infants’ basic care needs. Additionally, S1 placed infants on prolonged time-outs by confining them in highchair-style equipment and similar furniture as a form of discipline or punishment. Although no visible injuries were observed, this conduct posed an immediate risk to infants’ personal rights, health, and well-being.
