What the inspector wrote

The Type A deficiency listed on the attached LIC 809D page is being cited. LPA Dowling informed the Director that Type A citation(s) shall be posted for 30 consecutive days as there are immediate risk(s) to the health, safety, or personal rights of children in care. LPA Dowling also informed the Director to provide a copy of this licensing report that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A copy of this report, appeal rights, and Notice of Site Visit was provided. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Roshawn Hooper, Director. Page 2

This Requirement is not met as evidenced by: Based on IB investiagtion, interviews and review of records IB reveled that the violent altercation between Staff#1 and Staff# 2 posed a risk to children and other staff; therefore, the allegation of conduct inimical is substantiated.
