What the inspector wrote

Reporting Party indicated Child #1 (C1) wandered out of the facility through the front door unsupervised and found by another student's parent. Staff reported that child was never out of visual, nor did child exit front door to go outside. Per interview with Parent #1( P1), Parent that observed child by the door, child never left facility grounds and did not go outside, as staff intervened immediately upon P1 noticing Child #1 walking with his children by the front door. A full investigation was conducted which included observations and interviews. The information received did not reveal evidence that the allegation was true. The allegation is unfounded. Unfounded means that the allegation is false, could not have happened, and/or is without a reasonable basis. Child 1 did not wander out of the facility through the front door unsupervised. Child 1 was in the front area awaiting pick up. Administrator indicated a Plan of Correction was put in place to ensure each student has been accurately checked out, escorted by staff and dual announcement from intercom system and radio's with confirmation given before student is sent to front for checking out.. Exit interview was conducted with Roshawn Hooper, Administer and a copy of the report was provided. A Notice of Site visit was given and must remain posted for 30 days. Appeal rights were reviewed and provided. Page 2
