Based on record review, Staff #2 (S2) file did not include documentation of the required immunizations. Per licensee, S2 made appointment to acquire immunization records. Per licensee, S2 start date was 01/05/2026. LPA informed licensee that all employees must maintain documentation of immunizations against MMR, Tdap, and Influenza prior to employment at the facility.

Based on record review, Child #1 (C1) did not have an Individual Infant Sleeping Plan (LIC 9227) on file. Per licensee, parent received form but did not return to licensee. Per licensee, C1 start date at facility was 02/05/2026. LPA informed licensee that an Individual Infant Sleeping Plan must be completed and on file for all infants (12 months and under).

Staffing Ratio and CapacityThe capacity specified on the license shall be the maximum number of children for whom care may be provided at any one time. This requirement is not met as evidenced by:In the month of Jan.22 R&R subsidy Child care time sheets(CCRC) reflect licensee was caring for a total of 17 children at one time(2pm4pm) Monday - Friday, this is above the amount specified on the license. According to the current roster provided Licensee has 23 children enrolled. This is a type A deficiency that poses an immediate health and safe risk to children.

(a) Personnel records shall be maintained on each employee and shall contain the following information: (10) A signed and dated copy of the Notice of Employee Rights [LIC 9052, (Rev. 03/03)] as required by Section 102416(a) and Section 102417.This requirement is not met as evidenced by: Based on record review the licensee did not comply with the section cited above Employee Cox disclosed she does not have mandated reported training, CPR/First aid, Driver's license, auto insurance and is providing pick ups and supervision which poses/posed a potential health, safety or personal rights risk to persons in care.

Not visual observation was completed and no 15 min checks were observed. This possess an immediate risk to children in care.

(a) Each child receiving services shall have certain rights that shall not be waived or abridged by the licensee regardless of consent or authorization from the child's authorized representative. To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse, or other actions of a punitive nature, interference with eating, sleeping or toileting; or withholding shelter, clothing, medication or aids to physical functioning this requirement was not met by interview disclosures. licensee is restraining children on high chairs and using off limits room to punish children when not wanting to nap or not listening. This poses an immediate risk to children in care.

This requirement was not met by interviews conducted and documentation(child##2 positive results) and disclosures. This posses a potential risk to children in care.

All adults assisting at the facility or living at the facility must first obtain a fingerprint clearance. A civil penalty of $100.00, per day the person has been present, will be assessed. The first violation is subject to the penalty for up to five days. If there is a subsequent violation in a 12 month period, a civil penalty may be assessed up to 30 days. A civil penalty in the amount of $1,000.00 will be assessed. Please see Lic 421B... Criminal record clearance: Florencia Cox was observed supervising day care children and is not fingerprint clearanced. She stated that she has been assisting with the facility for a about a year.
