What the inspector wrote

On 7/23/2025 Licensing Program Analysts (LPAs) D. Valdez Santana and C. Sun conducted an unannounced Plan of Correction visit to follow up on the deficiency 102417(a) Operation of a Family Child Care Home issued on 05/23/2025. Upon arrival, LPAs met with co-licensee, Rasika Fernando. Co-licensee stated that the other co-licensee, Tharindu Fernando, was not present due to leaving out of the country on July 18, 2025. LPAs informed licensee about the purpose of the visit. Licensee guided LPAs on a tour of the facility and census was taken. Total census was 2 children and 1 staff. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPAs observed licensee has corrected the violation by returning to the facility and licensee understands that she is to remain within compliance of 102417(a) Operation of a Family Child Care Home regulation. During today’s inspection, LPAs observed that the licensee has returned to the facility and is present with the children. LPAs also obtained a written declaration from Licensee. By this visit and LPAs’ observations, the violation which was cited on 05/23/2025, is cleared. The licensee was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The exit interview was conducted with licensee, Rasika Fernando. The Licensee was provided a copy of appeal rights LIC 9058 and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. End of Report.
