(1)... physically check on the infant every 15 minutes.This requirement is not met as evidenced by: Based on observation and file review, C1 was asleep at 9:55 AM when LPA arrived. At 10:10 AM, LPA reviewed sleep log and observed that the log had not been filled out for 8/28/25. C1 woke up at 10:30 AM. Licensee confirmed that she had not completed the log today. LPA provided PIN 20-24-CCP-SP.

This poses a potential health, safety or personal rights risk to persons in care.

LPA observed that Child 2 was asleep in a baby swing when LPA arrived. Although he woke up shortly after, Licensee stated that Child 2 can only fall asleep on the swing.
