This optional toddler program shall be subject to the following basic conditions: (3) A ratio of six children to each teacher is maintained for all children in attendance at the toddler program...this requirement is not met as evidenced by: on 08/20/25, during LPA’s initial complaint visit, LPA observed a census of 14 children and 2 staff in the Early Preschool Classroom. However, a child enrolled in the Toddler Option program was included in the ratio that day, and a ratio of six children to each teacher should be maintained for all children in attendance at the toddler option program.

This requirement was not met as evidenced by: Multiple witness statements that staff do not ensure children’s hygiene needs are met while toileting in that potty training children are solely responsible for wiping themselves without staff supervision or assistance. This constitutes a potential health, safety, and/or personal rights risk to children in care.
