Annual compliance review inspection found all areas in compliance with no deficiencies. One hazard (glue traps near classroom entrance) was identified and immediately removed during inspection.
Case management follow-up to April incident. Staff member observed on 4/8/25 aggressively redirecting a child with harsh tone during dismissal. Internal investigation conducted; staff member removed from classroom and reassigned to older age group for new school year.
Lead testing results identified exceedances at two drinking water outlets. Facility implemented interim measures with covered/sealed outlets and water bottle provision pending installation of new drinking water fixtures.
Complaint investigation regarding hand-foot-mouth disease outbreak notification. Facility notified only classroom 3 parents of HFM cases but failed to notify classrooms 1 and 2. Facility also failed to report to Licensing. Allegation substantiated.
