Complaint investigation substantiated allegation of operating out of ratio. Facility staff records showed 3 out of 3 randomly selected dates with multiple instances of rooms left understaffed for 5+ minutes during busy drop-off times. Type A citation issued.

Unannounced case management visit found a subject child left unsupervised for approximately 2-3 minutes when they wandered from Twos classroom into adjacent Threes/school age classroom undetected. Cited as repeat Type A violation with $1,000 civil penalty issued.

LPAs confirmed that two children were left unattended in a classroom during morning recess. The children were later escorted out after staff realized they had been alone, and the case resulted in a Type A supervision violation and civil penalty.

Investigation of incident where staff member pulled child by arm in rough manner and forcibly sat child down. Type A violation issued for personal rights violation.
