    PERSONNEL REQUIREMENTS

    Based on record review and interview, the licensee did not comply with the section cited above in that 2/5 staff (Staff 4 [S4] & Staff 5 [S5]) did not have current EMSA approved Pediatric CPR/First Aid on file while S4 and S5 transported children alone, which poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on record review and interview, the licensee did not comply with the section cited above in that 1/5 staff (Staff 2 [S2]) did not complete renewal of mandated reporter training. S2 did have proof of completion of a different mandated reporter training certificate, however this certificate did not satisfy the requirements created by Assembly Bill 1207, which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA observed during record review the facility was not cited for a lead exceedance in 2022/2023. Director stated a retest of outlet identified with lead is being conducted.

    Based on record review, the licensee did not comply with the section cited above. Staff #4 did not have a Mandated Reported Certification current and availalbe for review. Per records obtained at the facility Mandated Reported Certificate expired 03/05/2020 which poses/posed a potential health, safety or personal rights risk to persons in care.

  • PERSONAL RIGHTS

    (a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. This requirement was not met as evidenced by: Personal Rights/ dignity of two children were violated by another child during an offsite activity organized by the facility. Incident(s) occurred in one of more occasion, in the same day. Children are still attending the facility. This poses an immediate risk to the health and safety and Personal Rights of children in care.
