What the inspector wrote

On date and time listed above, Licensing Program Analyst (LPA) Chase Atherton and Licensing Program Manager (LPM) Ana Noble arrived at the facility to conduct a Case Management-Legal/Non-compliance inspection and were granted entry by licensee Patricia Alarcon-Enriquez. LPA & LPM toured the entire facility, reviewed records, and observed and/or discussed the following: LPA and LPM provided Order to Licensee/Facility of Immediate Exclusion from Facility Letter addressed to Licensee and a copy of letter to Mr. Enriquez to the Ms. Alarcon-Enriquez during this inspection on this date. LPM read the letters aloud to the licensee during this visit, Licensee indicated understanding the information provided in the Immediate Exclusion Letter. The letters, in part, stated that Community Care Licensing has obtained information to support that the conduct of Gilbert Enriquez has been inimical to the health, morals, welfare, or safety of either an individual in or receiving services from this facility, or the people of the State of California while working at the facility. The California Department of Social Services (CDSS) has determined that the continued or future contact with clients or presence in any community care facility, child day care facility, residential care facility for the elderly, or any other facility licensed by CDSS, constitutes a threat to the health, morals, welfare or safety of the clients in care. The letter states that upon receipt of the notice, Gilbert Enriquez must remove himself from any contact with clients and not be physically present in any facility. This action is final until notification has been provided otherwise, in writing by CDSS. An appeal can be made against this decision in writing to Department of Social Services - Community Care Licensing. The appeal must be received within 15 days of the date of this letter. The letter is dated 5/6/2026. Licensee, Patricia Alarcon-Enriquez, stated that Gilbert Enriquez has not been present since March 9, 2026 and no longer lives at the licensed address/facility. A copy of the letter(s) were provided to the the licensee Patricia Alarcon-Enriquez, on this date. An exit interview was conducted and a copy of this report was provided to the licensee Patricia Alarcon-Enriquez on this date. A copy of this report must be made available to the public for three years. A notice of site visit was given to licensee Patricia Alarcon-Enriquez and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
