Annual required inspection of child care center with multiple classrooms operating at approved ratios. Sign-in/out procedures found incomplete and 2 staff members have expired Mandated Reporter training.

Office meeting to discuss deficiency corrections from prior citations. Facility had been cited for unattended child on playground (Type A) and teacher-child ratio violation (initially Type A, reclassified to Type B). Facility implemented corrective training and supervision procedures.

During the annual inspection, the analyst observed 77 children with 11 staff and later saw one staff member alone with 13 children in classroom 4B. The visit also confirmed the facility had completed lead testing and was remediating lead exceedances, while the rest of the walkthrough covered safety, records, food service, and emergency preparedness compliance.

Case management investigation of three self-reported incidents resulted in Type A deficiency citation for unsupervised children on playground. Two other incidents (allergic reaction and fall) determined to not warrant citations.
